What Are the Causes of Endometriosis?

The exact cause of endometriosis is unclear. Most expert physicians agree that there are a number of factors that ultimately lead to the development of endometriosis. Furthermore, the cause of endometriosis could vary from one individual to another. Endometriosis is thought to have a strong genetic component. In other words, most patients with endometriosis have a close relative with endometriosis. Another possible cause of endometriosis in some patients is retrograde menstruation. In retrograde menstruation, small portion of the lining of the uterus, which is called endometrium, migrates through the tubes into the abdominal cavity. The endometrium tissue can then adhere to the inside of the pelvis whereby triggering inflammation. It is ultimately this chronic inflammation that will result in pain and scar formation.

What Symptoms does Endometriosis Cause?

Endometriosis can have a wide variety of symptoms. Most patients with endometriosis report symptoms of:

  • Pelvic pain
  • Painful menstrual cycles
  • Painful vaginal intercourse
  • Pain during bowel movement

It is worth mentioning that most patients with endometriosis report having had very painful menstrual cycles since teenage years. Endometriosis can be a cause of infertility as well.

What are the Endometriosis Treatment Options?

There are two main treatment options available for endometriosis. One option is medical therapy and the other option is surgery. Medical treatment typically starts with prescribing birth control pills to suppress endometriosis. Birth control pills are generally effective for early stage endometriosis. If birth control pills or progesterone pills become ineffective, the physician would discuss and offer stronger medications such as Lupron or Orilissa. These medications are designed to create a state of menopause or estrogen deprivation in the body. Since estrogen is the main “fuel” for endometriosis, estrogen depletion would suppress the activity of endometriosis and put the disease in remission. Unfortunately, some patients cannot tolerate the side effects of estrogen depletion such as mood changes, insomnia, and joint ache to name a few. On the other hand, some patients do very well on Lupron and Orilissa and experience significant improvement of their pelvic pain.

Most patients with endometriosis do well with a combination of surgery and medical treatment, and go on to lead active, pain-free productive lives. Most infertile couples are able to conceive after endometriosis treatment, either spontaneously or with medical assistance. Diagnosis of endometriosis should not foster self-pity or helplessness. It is very important for the endometriosis patient to assume an active role in her treatment and seek the services of a health professional who will listen to her concerns and guide her through treatment options.
