What Is a Myomectomy?

A myomectomy is a surgical procedure that removes uterine fibroids while preserving the uterus. Uterine fibroids are noncancerous growths that typically develop during the childbearing years. These growths can distort or enlarge the uterus, potentially making pregnancy more difficult by interfering with embryo implantation.

In many cases, fibroids do not cause noticeable symptoms and are discovered during a pelvic exam or ultrasound. When symptoms do occur, they may include:

  • Abnormally heavy menstrual bleeding
  • Frequent urination
  • Constipation
  • Pelvic pain

Dr. Ali Ghomi may recommend a myomectomy if you are planning to conceive or if fibroids are affecting your fertility.

Learn About Fibroid Removal and Understand Your Options

Move Freely Through Life

Fibroids are benign (noncancerous) tumors found inside or on the walls of the uterus. While some fibroids are symptom-free, others can cause significant issues such as pelvic pain, heavy periods, infertility, or miscarriage.

Often, fibroids are detected during a routine pelvic exam. Your doctor may order additional imaging tests, such as ultrasound, to determine their size and location, which helps guide treatment decisions.1

Understanding Your Options

Depending on symptom severity, your doctor may recommend monitoring your condition or prescribing medications to shrink the fibroids. Other non-surgical treatments include thermal ablation (heat or cold) or uterine artery embolization, which cuts off the fibroids’ blood supply.1, 2

If symptoms persist or affect your quality of life, your doctor may suggest a hysterectomy to remove both fibroids and the uterus.

A myomectomy is another option that removes only the fibroids, keeping the uterus intact. This allows the possibility of future pregnancies. However, because the uterus remains, new fibroids may develop later.1

Types of Myomectomy Surgery

Myomectomies can be performed through open or minimally invasive surgery. In an open myomectomy, the surgeon makes a large incision in the abdomen to access the uterus and remove fibroids using handheld instruments.

Minimally invasive options include:

  • Vaginal (Hysteroscopic) Myomectomy – Performed through the vagina using a lighted scope and surgical tools. This method involves no incisions but depends on fibroid location and size.
  • Laparoscopic or Robotic-Assisted Surgery – Performed through a few small incisions. Surgeons use long-handled instruments and a camera to guide the procedure, often using da Vinci® technology for enhanced precision.

How the da Vinci System Works

The da Vinci surgical system enables robotic-assisted surgery. Your surgeon operates from a console, using tiny wristed instruments that mimic hand movements with enhanced precision. A high-definition 3D camera gives the surgeon a magnified view inside your body.

Every movement your surgeon makes is translated in real time to the instruments inside your body, allowing for precise and minimally invasive surgery.

Please note: Intuitive does not provide medical advice. You and your doctor will determine whether a da Vinci procedure is appropriate. Always ask about your surgeon’s training, experience, and outcomes.

What Are the Outcomes of Surgery with the da Vinci System?

Surgical outcomes vary by surgeon. Ask your doctor about the following:

  • Length of hospital stay
  • Complication rate
  • Readmission rate within 30 days
  • Reoperation rate
  • Blood transfusion or blood loss
  • Conversion to open surgery
  • Length of procedure
  • Mortality rate

Discuss all important outcomes with your doctor. All surgeries carry risks. You can read more about myomectomy-related risks here.
